One of the wonderful pieces of information that came out last February when Nancy and David Ludescher and Susan Lohmann spoke at St. Dominic’s about end-of-life issues was that there was a Minnesota Catholic Health Care Directive and that one can ask for it at the hospital or prepare beforehand by requesting a free copy from the Minnesota Catholic Conference of Bishops website.  There is also a wonderful guide to go with it.
